Ageing is the primary cause of tired-looking eyes, primarily due to the depletion of collagen and fat padding that occurs over time. Collagen is crucial for the structural integrity of muscles, skin, and other tissues throughout the body. Reduced collagen levels lead to diminished skin elasticity and muscle tone, resulting in sagging, lines, and wrinkles. The thin skin under the eyes makes these effects particularly noticeable in that area. This sagging and fine lines can be more pronounced under and around the eyes.

As skin weakens and sags around the eyes, a hollow depression called tear trough hollowing can form, where fat pads beneath the eyes may descend, creating the appearance of under-eye bags. These bags are often most noticeable in the morning due to fluid retention overnight. With age, lymphatic drainage in this area slows down, contributing to the appearance of under-eye bags, which are typically a combination of descended fat pads or herniated fat pads and swelling from sluggish lymph drainage.

Other factors such as allergies, fluid retention, smoking, infections, thyroid issues, eye strain, genetic predisposition, and inadequate sleep can exacerbate dark circles, puffiness, fine lines, and eye bags. In most cases, eye bags are harmless and are primarily a cosmetic concern.
